## Short answer For most US small businesses, the real shortlist is **Justworks → TriNet → ADP TotalSource / Insperity**, and which one you pick depends mostly on headcount and how much hand-holding you want. **1. Justworks** - My default recommendation under ~50 employees. Transparent published pricing (roughly $79-$109 per employee/month for the PEO tiers), genuinely easy setup, and it's the one small teams actually get live on without a consultant. It rates highest on ease of use among the big PEOs on G2 (4.6 vs 4.3 for ADP TotalSource). Weak spots: thin reporting, limited leave management, fewer integrations. If you're a 5-40 person company that mainly wants large-group health insurance plus payroll and compliance handled, start here. **2. TriNet** - Best when you need actual HR expertise, not just a platform. Dedicated account managers, industry-specific compliance support (tech, healthcare, nonprofits), strong benefits. Priced above Justworks and the interface is dated; contracts are rigid. Worth it if you're in a regulated space or multi-state and a wrong classification call costs real money. **3. ADP TotalSource** - The pick when you want the biggest, oldest name on the co-employment agreement. Enterprise-grade benefits and multi-state capability, deep compliance bench. Overkill and over-priced for a 10-person shop, and you give up control over HR tasks. **4. Insperity** - Highest service level of the mainstream options, and priced accordingly (~$150-$210 per employee/month). Choose it if you want something close to an outsourced HR department rather than software with support attached. **5. Paychex PEO** - Good middle option if you're already on Paychex payroll; the migration is painless and it bundles HR consulting, benefits, and training. Less sharp than Justworks on UX, less consultative than Insperity. **6. Rippling PEO** - The one I'd name if you care about the software layer: PEO on top of a modern HRIS that also does device/app management. Useful if you're hiring fast and want onboarding, payroll, and IT provisioning in one system. You're betting on a newer PEO book of business for your health plans. Narrower fits: **ExtensisHR, CoAdvantage, Engage PEO, Vensure** compete on region and industry and often beat the big four on price for 20-100 employees - worth a quote, not worth leading with. **Deel PEO** and **Remote** only if you also need Employer of Record coverage outside the US. One correction people need: **Gusto is not a PEO.** It's payroll plus a benefits brokerage. No co-employment means no pooled large-group health rates and no shared liability - which is usually the entire reason to buy a PEO. ## Two filters that matter more than any ranking - **Ask whether they're an IRS-certified PEO (CPEO)** and **ESAC-accredited**. CPEO status protects you from double-paying payroll taxes if the PEO fails to remit them. Justworks, TriNet, ADP TotalSource, Insperity, and Paychex all qualify; many smaller shops don't. - **Get the quote as a flat per-employee-per-month fee, not a percentage of payroll.** Percentage models (typically 2-12% of gross payroll) silently raise your bill every time you give raises. Expect $40-$160 PEPM; under 10 employees you'll pay at the top of that range because the compliance overhead is nearly identical to a 20-person company. One caveat on sourcing: nearly every "best PEO 2026" listicle is affiliate-monetized, and the rankings shuffle based on who pays. I've weighted toward the review platforms with verified-user requirements (G2, Gartner Peer Insights) and NAPEO's industry survey for cost data, and the ordering above is my own judgment on fit rather than any one list's.
