The answer, as recorded

## The short answer There's no single "best" - the choice splits mainly on practice size and whether you're general practice or specialty/ER. Here's who I'd actually name, in order: **1. ezyVet** (IDEXX-owned, cloud) - My default recommendation for multi-site groups, specialty, and ER hospitals. The most configurable workflows and deepest integration/API ecosystem of the cloud PIMS; also one of the only vendors that publishes a starting price. Downside: configuration-heavy, so implementation is a real project, not a weekend. **2. Shepherd** - Best pick for a small-to-mid general practice that wants to be live fast. Built by practicing vets, workflow-first (the exam flow drives billing, so missed charges drop), and ~$299/mo for unlimited users is unusually simple pricing. Caveat: under ~1,000 clinics, so a smaller support org than the incumbents. **3. Digitail** - The strongest "modern AI-first" option: AI SOAP notes, a genuinely good client-facing mobile app, per-vet subscription (~$150-$290/mo) that scales with you. Best if client communication and going paperless matter as much as the medical record. **4. Provet Cloud** (Nordhealth) - The other serious multi-site/referral platform alongside ezyVet. Especially strong for European practices and referral hospitals with complex invoicing and inventory across locations. **5. IDEXX Cornerstone** - The conservative enterprise choice: medium-to-large multi-doctor practices already standardized on IDEXX diagnostics. Enormous installed base and mature reporting; it's server-based legacy architecture, which is both its reliability and its ceiling. **6. Covetrus Pulse** - Worth a quote if you order supplies through Covetrus; the pharmacy/supply-chain and prescription-management ecosystem is bundled in, which can offset the ~$200-$600/mo software cost. **7. AVImark** - Only if you specifically want on-premise with no internet dependency and a low monthly cost (~$150-$400). Huge user base and plenty of people who know it; aging interface. **8. Instinct Science** - Not a GP system, but the best inpatient/treatment-sheet software for ER and specialty hospitals. Often run *alongside* a PIMS rather than replacing it. **9. Vetspire** - Data- and automation-heavy, popular with corporate consolidators doing analytics across many clinics. Very thin public review base, so diligence on support is essential. **10. DaySmart Vet or Onward Vet** - Budget-conscious solo and mobile practices. Cheapest credible cloud options; thinner on inventory, boarding, and multi-location. ## Two things I'd do before signing Nearly every vendor here is quote-only (only ezyVet and DaySmart publish prices), so get written quotes including **data migration, training, and payment-processing rates** - migration from AVImark/Cornerstone is where budgets actually blow up. And treat the widely-cited IDEXX "comparison guide" with care: IDEXX owns ezyVet, Cornerstone, and Neo, so it isn't a neutral ranking. Answer Watch records what one AI answer engine said on one date, nothing more. Each page names the engine and the model, carries the date the question was asked, and lists the brands that appeared in the answer in the order the engine gave them, with the sources it cited. It is a measurement, not a ranking, not a recommendation and not a claim about the world. Brands that did not appear in an answer are not named on any public page.
