Here's who I'd actually name, in order. ### If you want billing + EHR from one vendor (most clinics) 1. **Tebra** (formerly Kareo) - my default recommendation for solo and small practices (1-5 providers). Easiest onboarding, good claim-tracking and denial visibility, and priced for small teams. It's the one that consistently gets named "best for tracking claims." Weakness: it doesn't publish a first-pass clean-claim rate, so you're trusting the pitch more than the number. 2. **athenahealth (athenaOne / athenaCollector)** - the strongest choice once you're past ~5 providers or multi-specialty. Its real moat is the shared payer-rules library across its whole client base, which is why it posts a ~1.6% front-end rejection rate. Pure percentage-of-collections pricing, so incentives are aligned. Most expensive of the group, and the contracts are sticky - read the term and termination clauses carefully. 3. **AdvancedMD** - pick this if you have (or want) your own biller in-house and need real tooling: claim scrubbing edits, denial worklists, serious reporting. Best fit for independent multi-provider groups that want a hybrid - software plus partial outsourcing. 4. **CareCloud** - solid mid-market alternative, publishes its clean-claim metrics, decent if athenahealth quotes too high. 5. **eClinicalWorks RCM** - worth a quote mainly if you're already on eCW. Huge install base, but the UI and support reputation are weaker than the above. ### If you want to keep your existing EHR 6. **Omega Healthcare** - won 2026 Best in KLAS for Ambulatory RCM Services (EHR-Agnostic) with a 92.2 score. This is the one independently-verified signal in a market full of self-published rankings. 7. **R1 RCM** - held that same KLAS category five straight years before Omega. Built for larger groups and health-system-affiliated practices; probably overkill under ~20 providers. ### Specialty-specific (usually beats a generalist) 8. **WebPT** for PT/OT/SLP, **SimplePractice** for behavioral health solo/small, **Alma** or **Headway** if you're a therapist who mainly wants someone else to own credentialing and insurance entirely. Specialty billers know your payer quirks and modifier rules in a way a generalist won't. ## What actually matters when you negotiate - **Expect 4-10% of net collections; 6-7% is the normal quote** for general practice.
